In most cases, it's the water — not the fish

When fish die repeatedly for no obvious reason, the cause is almost always the water chemistry rather than disease, bad luck, or a defective fish. Specifically, the problem is usually a failure of what aquarists call the nitrogen cycle. Fish produce ammonia constantly through their waste and respiration. In a healthy, established aquarium, colonies of beneficial bacteria convert that ammonia first into nitrite (also toxic) and then into nitrate (far less harmful). When those bacterial colonies haven't developed — or have been disrupted — ammonia and nitrite accumulate to lethal concentrations in water that still looks perfectly clear.

This is so common it has a name: new tank syndrome. It kills fish in brand-new aquariums that haven't been properly cycled, but it can also strike established tanks after a large water change, filter cleaning, or medication treatment that wipes out the bacterial colonies. Because the water appears clean, owners rarely suspect it — and the fish are often dead before anyone thinks to reach for a test kit. Chlorine and chloramine in tap water are a second major silent killer: added by municipalities to make water safe for humans, these chemicals are acutely toxic to fish gills and also destroy the beneficial bacteria your filter depends on.

Temperature shock, incompatible tank mates, and overcrowding round out the usual suspects. Fish are cold-blooded, so sudden swings of even a few degrees — during water changes or when moving fish — can be fatally stressful. Overcrowding accelerates ammonia production faster than any filter can handle. These factors often interact: a slightly overstocked tank that was stable for months can crash after a routine cleaning that removes too much of the bacterial bed.

    I went through about 20 fish over two months in a brand new 20-gallon tank. Changed the water religiously, bought "good" fish from three different stores — nothing worked. Finally bought a test kit on a whim and my ammonia was off the charts. The tank had never cycled.     My tank had been running for almost a year with no problems, then I did a big clean — scrubbed the filter, replaced most of the gravel — and within a week three of my tetras were dead. I had no idea that cleaning the filter could destroy the bacteria. I now only ever rinse the filter media in old tank water, never tap water, and I do partial gravel cleans, never the whole thing at once.

    New fish kept dying within 48 hours of coming home while my older fish were totally fine. Turns out the fish store kept their tanks at 78°F and I was acclimating mine in a bag for only about five minutes before releasing them into 74°F water. The temperature shock was killing them. I now float the bag for 20 minutes and slowly add tank water before releasing. No losses since I changed that one thing.
