Why your fridge runs but stays warm
Your refrigerator cools by circulating refrigerant through a closed loop — the compressor pressurizes the refrigerant, the condenser coils (usually on the back or underneath) release that heat outside the cabinet, and the evaporator coils inside absorb heat from the air in your fridge. When you hear the fridge running, you're hearing the compressor or the fans doing their jobs — but "running" and "cooling" are two separate things. The motor can spin perfectly while any one of several components in the cooling loop fails silently.
The most common culprits are: condenser coils so coated in dust and pet hair that they can't shed heat; a failed evaporator fan motor that stops circulating cold air through the compartment; a frosted-over evaporator (caused by a defrost system failure) that blocks airflow entirely; or a worn start relay that prevents the compressor from actually building pressure even as it tries to cycle. Each of these produces the same symptom — the fridge runs, but nothing gets cold — yet each has a completely different fix.
Less commonly, the problem is a refrigerant leak or a failed compressor. These are the expensive outcomes, and they're also the ones you typically can't see. The good news is that most running-but-not-cooling situations are caused by the simpler mechanical failures listed above — and those are diagnosable at home before you spend a dollar on a service call.
A running fridge that won't cool isn't always the same problem
The way the problem shows up can tell you a lot about what's actually failing — check whichever description fits your situation most closely.
What happens if you leave it alone — and how fast it can get expensive
The immediate concern is food safety. A refrigerator that isn't maintaining 40°F (4°C) or below is a food-safety hazard — bacteria that cause illness multiply fastest in the 40–140°F range, and you often can't smell or see the difference between safe and unsafe food once temperatures have climbed. The longer-term concern is cost: problems like dirty condenser coils or a failed start relay are inexpensive to fix, but leaving them unaddressed forces the compressor to work much harder than it was designed to, which can push a cheap repair into an expensive compressor replacement over time. Compressor failures, when they happen, often cost more than the refrigerator is worth — particularly on older units.
The USDA Food Safety and Inspection Service states that perishable food in a refrigerator that has lost cooling is only safe for up to 4 hours — provided the door stays closed. After that point, meat, dairy, eggs, and cooked leftovers should be discarded. A $10 refrigerator thermometer can tell you exactly where temperatures stand right now, and whether you're already past that window.

Ours stopped cooling on a Saturday morning — compressor was running nonstop but the interior was 58°F. I pulled the fridge out and the condenser coils looked like a cat had been living back there for five years. Vacuumed everything out, and within two hours it was back down to 37°F. Felt ridiculous how simple it was after a weekend of panic.

My freezer was fine but the fridge section was 55°F. The fan between the freezer and fridge compartments had stopped spinning — you could hear nothing when the compressor ran. Ordered a replacement evaporator fan motor for $28 on the manufacturer's parts site and swapped it in about 20 minutes. Would have been a $200+ service call. The tricky part was just finding the model number on the inside wall to order the right part.

Ours was clicking every few minutes and not cooling at all — I read that this clicking pattern usually means the start relay is failing. Pulled the relay off the compressor (it just pops off), shook it, and it rattled — apparently dead relays rattle and working ones don't. Replaced it for $12. It's been running perfectly for six weeks since. I did throw out about $80 worth of groceries though, so catch it early if you can.
