Why two drugs that are each safe on their own can become a problem together
Most drugs are processed by your liver using a family of enzymes called cytochrome P450 (CYP450). The trouble starts when one drug either speeds up or slows down those enzymes. If drug A slows the breakdown of drug B, drug B accumulates in your blood to levels far higher than your doctor intended — which can turn a normal dose into a toxic one. The reverse is also dangerous: if drug A accelerates the metabolism of drug B, drug B is cleared before it can work, leaving you under-treated.
Interactions don't always happen in the liver. Some drugs compete for the same receptor sites, amplifying or canceling each other's effects. Others change the acidity of your stomach or gut in ways that prevent a second drug from being absorbed properly. And some combinations affect the electrical conduction of your heart, raising the risk of dangerous arrhythmias even when neither drug alone would cause that problem. The mechanism matters because it determines both the severity and the timing — some interactions are immediate, others build up over days or weeks.
Supplements and over-the-counter medications are part of this picture too, and they're the most frequently missed. St. John's Wort is one of the most potent enzyme inducers known — it can cut blood levels of warfarin, birth control pills, and certain HIV medications by half. Grapefruit juice blocks a key CYP3A4 enzyme and can dramatically amplify the effect of statins, calcium channel blockers, and some immunosuppressants. If you only check your prescription drugs against each other, you're only seeing part of the risk.

I was taking ibuprofen pretty regularly for back pain without thinking much about it — just OTC stuff. Then my doctor ran labs and my kidney numbers had gotten worse.
