The mold came back within about four months — smaller, but back. Turns out I had a slow drip from a supply line I hadn't noticed because I was looking at the floor level, not the pipes. Once the plumber fixed the drip and I retreated the wood and let it dry properly, the problem finally stopped. The lesson for me: when you inspect, look up at the pipes and subfloor, not just at the ground. It's easy to only look at what's at eye level when you're crawling around in there.
