Cholesterol is more complicated than "high is bad" — and your diet is only part of the picture
Cholesterol is a waxy substance your body produces naturally and also absorbs from food. It travels through your bloodstream bound to proteins called lipoproteins, and the type that matters most for heart disease risk is LDL (low-density lipoprotein) — sometimes called "bad" cholesterol — because at elevated levels it can accumulate in artery walls and contribute to plaque build-up. HDL ("good" cholesterol) works in the opposite direction, helping to ferry cholesterol back to the liver. Your total cholesterol number on its own is actually a poor predictor of risk; the ratio of LDL to HDL, your triglyceride level, and the particle size of your LDL all paint a more accurate picture.
Here's what most doctors don't have time to explain in a ten-minute appointment: roughly 70–80% of the cholesterol in your blood is produced by your own liver, and the liver adjusts its output in response to how much dietary cholesterol you eat. For most people, this means eating cholesterol-rich foods (like eggs) has a surprisingly modest effect on blood cholesterol. What does have a more significant effect is the mix of fats and carbohydrates in your diet — specifically, saturated fat from red meat and full-fat dairy raises LDL more than dietary cholesterol itself does, while refined carbohydrates and added sugars raise triglycerides and can lower HDL.
Genetics also play a meaningful role. Familial hypercholesterolaemia — an inherited condition that impairs the liver's ability to clear LDL — affects roughly 1 in 250 people and causes stubbornly high LDL regardless of diet. If your cholesterol has been consistently elevated since a young age, or if close relatives have had early heart disease, this is worth raising explicitly with your doctor. Diet alone is rarely sufficient in that scenario, and that's not a personal failing — it's biology.

Sustained high LDL cholesterol is one of the better-established risk factors for cardiovascular disease, including heart attack and stroke. But "risk factor" is not the same as "destiny." Your absolute risk — the number that actually matters for deciding whether and how urgently to act — depends on your full cardiovascular profile: your age, blood pressure, blood sugar, smoking status, family history, and whether you already have any arterial disease. A 55-year-old non-smoker with no other risk factors and modestly elevated LDL is in a very different position from a 55-year-old with high blood pressure, prediabetes, and a parent who had a heart attack at 50. Both may have the same cholesterol reading on paper.
The good news is that diet can meaningfully move the needle for many people — not by following generic "eat less fat" advice, but by making targeted, evidence-backed changes. Studies consistently show that the right dietary changes can reduce LDL by 10–30% in people without familial hypercholesterolaemia, which is a clinically significant shift. That's real, measurable protection — and it doesn't require eating food you hate.

Diagnosed with familial hypercholesterolaemia at 38. I want to be honest here: for me, diet alone was never going to be enough, and it took me a couple of years to accept that. I eat genuinely well — Mediterranean-style, very little processed food — and my LDL on diet alone was 190. With a low-dose statin it's 95. I still think the dietary changes matter for overall health and they did bring my triglycerides down, but if your numbers are very high and your family history is bad, don't beat yourself up that food isn't fixing it. Some of this is genetic and medication is a legitimate tool.

My cholesterol went up after I went low-carb and added a lot of butter and cream. My HDL improved and my triglycerides dropped, but my LDL shot up. I've since learned this is a recognized pattern — some people are "hyper-responders" to saturated fat. Swapping most of the butter for olive oil and the cream for full-fat yogurt brought my LDL back down while keeping the other improvements. It's frustrating that different dietary approaches can do opposite things for different people, but knowing which camp you're in does help you make sense of your results.
