Your chest is a crowded neighborhood — many structures can hurt
The chest cavity contains your heart, yes, but also your lungs, esophagus, stomach's upper portion, a complex network of ribs, cartilage, and intercostal muscles, and a dense web of nerves — including the vagus nerve, which is extraordinarily sensitive. When any of these structures become irritated, inflamed, or under pressure, they produce pain that the brain has no reliable way to label as "this organ, specifically." The result is chest pain that feels alarming and real, but originates nowhere near your heart.
The most common non-cardiac culprits, in rough order of frequency, are gastroesophageal reflux disease (GERD) and esophageal spasm, musculoskeletal inflammation (particularly costochondritis — inflammation of the cartilage connecting your ribs to the breastbone), anxiety and panic disorder, and pulmonary causes like pleuritis or a resolving respiratory infection. Each of these has a distinct pattern, and once you understand the patterns, the picture usually clarifies considerably.
What makes this confusing is that several of these conditions can mimic cardiac pain with surprising fidelity. Esophageal spasm, in particular, can produce a crushing, pressure-like sensation that responds to nitroglycerin — the same drug used to treat angina — which led to significant diagnostic confusion for decades. The mechanism is different, but the nerve pathways overlap. This is not a reason to panic; it is a reason to pay careful attention to the surrounding context of your pain.
Non-cardiac chest pain doesn't always feel the same way

Most causes are benign — but the ones that aren't require fast action
The honest truth is that the overwhelming majority of people reading this have a manageable, non-life-threatening cause. Acid reflux, costochondritis, and anxiety-related chest pain are not medical emergencies, and they respond well to treatment. Left unaddressed, GERD can progress to esophageal damage over years; untreated anxiety can significantly erode quality of life. But neither will kill you tonight. What matters is distinguishing these patterns from the minority of cases where something genuinely urgent is occurring — because the consequences of missing a cardiac event, a pulmonary embolism, or a spontaneous pneumothorax are severe and time-sensitive. The warning signs are specific, learnable, and worth knowing cold.
Call emergency services immediately if your chest pain is crushing or pressure-like, radiates to your left arm, jaw, or back, comes with sweating, nausea, or sudden breathlessness, or appeared abruptly at rest with no obvious trigger. Mine turned out to be costochondritis after a particularly brutal week at the gym. The thing that identified it for me was that I could press on my sternum and reproduce the exact pain — like, right there, I'm doing it, that's the spot. My doctor said it was textbook. Anti-inflammatories and rest sorted it out. The frustrating part was that it took about three weeks to fully resolve and would flare up if I overdid it on upper body work.

I had chest tightness for months before I connected it to anxiety.
