Why the shampoo isn't working — and why it's not your fault
Head lice are tiny, wingless parasites that live on the human scalp and feed on blood. They cannot jump or fly — they spread through direct head-to-head contact, and occasionally through shared hats, brushes, or pillows. An infestation typically starts with a single fertilized female louse, which immediately begins laying eggs. One female lays between 50 and 150 eggs over her four-week life. Each egg — called a nit — is cemented to a hair shaft close to the scalp with a biological glue that is remarkably strong and water-resistant. The nits are small and pale brownish in color, positioned near the base of the hair shaft where body heat keeps them at the right temperature for development.
Over-the-counter lice shampoos contain permethrin — a synthetic pyrethroid pesticide. These products have never reliably killed nits. They were designed to kill live lice only, with label instructions telling you to repeat treatment in 7–10 days to catch the newly hatched generation. But now there is a second, more serious problem layered on top of that. A study published in the Journal of Medical Entomology found that 98% of lice samples collected across 48 states carry a genetic mutation — known as kdr (knockdown resistance) — that makes them essentially impervious to permethrin and related pyrethroids. Earlier research by the American Chemical Society confirmed pyrethroid resistance in lice populations across at least 25 states. The active ingredient in Nix. The active ingredient in Rid. The entire class of chemicals every mainstream lice treatment uses.
These are what researchers call "super lice." They are not a different species — they look and behave identically to ordinary lice. As of 2022, super lice have been confirmed in all 50 states. If your child has lice today, there is a very high probability that what you are dealing with is resistant to anything sold in a pharmacy — and that repeated applications are adding chemical exposure without solving the problem.
Resistant infestations don't all look the same
The same underlying problem — resistant lice combined with undissolved nit glue — shows up differently for different families.
Repeated treatment with products that don't work isn't harmless
Head lice cause itching and scalp irritation but do not transmit disease. The real harm of a resistant infestation is that it compounds quickly and the standard response — more of the same treatment — may not be neutral. A study presented at the Endocrine Society's 2017 annual meeting analyzed 463 boys ages 9 to 16 and found that a 10% increase in urinary levels of 3-PBA — the metabolite produced when pyrethroids break down in the body — was associated with a 73 to 110 percent increased likelihood of advanced genital development. Separate research has linked maternal pyrethroid exposure to behavioral problems in children. Retreating repeatedly with products that are likely not working anyway adds pesticide exposure without adding benefit.
At the same time, the infestation itself grows. Every undetected nit that survives treatment hatches within 7–10 days, matures into an egg-laying adult within two more weeks, and the cycle restarts — from the same head you thought you treated.
Standard over-the-counter treatments have never been formulated to dissolve nit glue. A female louse can lay up to 150 eggs over her lifespan — all cemented firmly to the hair shaft. Even a treatment that kills every live louse leaves behind a full generation of eggs waiting to hatch. Without something that specifically addresses the glue, re-infestation from your own child's head is nearly guaranteed regardless of which shampoo you use.

What nobody told me: the white specks I kept finding after treatment were nit shells still cemented to the hair shaft. The shampoo killed some live lice but did nothing about the eggs. They were just sitting there waiting to hatch. I had to research this myself to understand what I was actually dealing with.
