Refusing statins because of muscle pain you haven't yet experienced — Fear of side effects is the leading reason people decline or discontinue statins, but the SAMSON trial showed that 90% of muscle symptoms patients attribute to statins are actually present at the same rate on placebo — meaning most people who think they can't tolerate statins pharmacologically, actually can.

Focusing on your total cholesterol or LDL number in isolation — A high LDL number without context tells you very little about whether you need a statin; a 60-year-old male smoker with diabetes and an LDL of 110 has a far higher absolute risk than a 45-year-old non-smoking woman with an LDL of 160, yet the latter's number looks worse on paper. Risk calculators exist for exactly this reason.

Taking supplements marketed as "natural statins" instead — Red yeast rice contains monacolin K — which is chemically identical to lovastatin — and has similar efficacy and similar side effect risks, but with none of the quality control, standardised dosing, or safety monitoring of pharmaceutical statins. It is not a safer alternative; it's an unregulated version of the same drug.

Accepting the prescription without asking about your absolute risk reduction — Relative risk figures like "reduces heart attack risk by 36%" sound dramatic but can represent a very small absolute benefit if your baseline risk is low. Always ask: "What is my absolute risk reduction over 10 years?" and "What is the number needed to treat?" Those answers will tell you whether the trade-off makes sense for you specifically.

I tried the lifestyle route for six months — I was genuinely disciplined about it. My LDL improved a bit but my risk score barely moved because my age and family history are baked in and can't be changed. My doctor showed me the NNT for someone with my profile and it was pretty compelling — about 1 in 40 over 10 years avoiding a major event. I started atorvastatin. I did get some mild achiness in my legs for the first three weeks but it went away on its own. Wish I hadn't delayed quite as long but I don't regret doing the due diligence first.
