Why "normal" labs don't always mean you feel normal
Hypothyroidism is typically treated with levothyroxine, a synthetic version of T4 — one of the two main thyroid hormones. The logic is straightforward: your thyroid isn't making enough T4, so you replace it. But your body must then convert that T4 into T3, the active hormone that your cells actually use for energy, metabolism, mood regulation, and dozens of other functions. In many people, that conversion is impaired — meaning T4 levels can look adequate while T3 remains chronically low, and symptoms persist in full force.
The standard test used to monitor thyroid treatment — TSH, or thyroid-stimulating hormone — measures a pituitary signal, not the hormone levels in your tissues. It's possible for TSH to sit within the reference range while your Free T3 is in the basement. A 2019 study published in the Journal of Clinical Endocrinology and Metabolism found that approximately 15% of patients on levothyroxine had persistently low T3 levels compared to people with naturally functioning thyroid glands — a gap that TSH alone would never reveal. For those patients, the drug is doing its job on paper, but not in the body.
There's also the matter of what caused your hypothyroidism in the first place. The most common cause in developed countries is Hashimoto's thyroiditis — an autoimmune condition in which the immune system attacks the thyroid gland. Levothyroxine replaces the hormone your damaged thyroid can no longer make, but it does nothing to address the ongoing autoimmune process. Inflammation, immune activation, and intestinal permeability issues that often accompany Hashimoto's can drive fatigue, pain, and cognitive symptoms entirely independent of your TSH number. Treating the hormone deficiency is necessary — but for many people, it simply isn't sufficient.
Persistent hypothyroid symptoms aren't one single problem
People who still feel unwell on thyroid medication usually fall into one of several overlapping patterns — and identifying yours matters, because the underlying cause shapes the solution.
Undertreated hypothyroidism has real, compounding consequences
This isn't a matter of learning to live with tiredness. Chronically low thyroid function — even "subclinical" hypothyroidism with a borderline TSH — is associated with elevated cardiovascular risk, impaired cognitive function, worsening depression, and metabolic disruption that becomes harder to reverse the longer it persists. If your current treatment isn't fully resolving your symptoms, that's a signal worth taking seriously, not a reason to lower your expectations for how you should feel.
The good news is that this is a solvable problem for most people. The evidence points toward specific, testable causes — suboptimal Free T3, nutrient deficiencies (particularly selenium, iron, and vitamin D), unmanaged autoimmunity, and cortisol dysregulation — each of which has targeted interventions. The difficulty is that standard care rarely looks for them.
A 2020 survey of nearly 12,000 people with hypothyroidism published in Thyroid found that those taking levothyroxine alone reported significantly worse quality of life, more fatigue, and more psychological distress than people with normally functioning thyroid glands — even when TSH was within the reference range. The gap was largest in people with Hashimoto's. The authors concluded that TSH normalization alone is an insufficient treatment target for a substantial portion of patients.

My doctor added a small dose of liothyronine and within six weeks I felt like myself again for the first time since my diagnosis. My situation turned out to be Hashimoto's with severe gluten sensitivity I didn't know about. Even with optimized thyroid levels, I kept having flares. Going strictly gluten-free was the piece that finally calmed things down — my antibody levels dropped by more than half over a year. I know it sounds like a cliché at this point, but for me the inflammation was the real driver and the thyroid numbers were just a symptom of that.

I want to share a less triumphant experience because I think it's useful too. I've tried T3 combination therapy, desiccated thyroid, an elimination diet, and nearly every supplement recommended in thyroid communities. Some things helped a bit, nothing fixed it completely. What has made the biggest single difference for me is finally getting evaluated for sleep apnea — it was severe and completely undiagnosed. Turns out untreated sleep apnea mimics and worsens every hypothyroid symptom there is. Don't assume thyroid is the whole story.
