These approaches are almost universal among worried parents — but the research is consistently clear that they either don't help or actively make things worse.

Cold-water or alcohol sponging — A Cochrane review found that sponging provides only a brief, modest temperature drop compared to antipyretics and causes significant discomfort; alcohol sponging additionally carries a risk of skin absorption and toxicity in children and should never be used.

My 4-year-old hit 103.8°F on a Sunday night and I was in full panic mode. I gave her the right dose of children's ibuprofen based on her weight (finally looked that up instead of guessing), kept her in a light t-shirt, and offered water and diluted juice constantly. By morning she was down to 101°F and by that afternoon she was asking for her tablet back. The key shift for me was focusing on whether she seemed alert and engaged rather than just the number — she was, so I didn't go to the ER, and that was the right call.

We have a 10-week-old, so when she hit 100.6°F at 2am we went straight to the ER — didn't wait, didn't call, just went. I'd read about the under-3-months rule and I'm glad we did. It turned out to be a minor UTI that needed antibiotics, and the ER team said catching it that early was exactly right. Do not wait-and-see with a baby that young. The rule exists for a reason.

I tried the "just monitor" approach with my 3-year-old's 102°F fever — he seemed okay but kept waking at night and was clearly miserable. I eventually gave him acetaminophen after about 18 hours and he slept properly and woke much more himself. I think I overcorrected away from treating fevers after reading that they don't need to be suppressed, but I missed the point: the medication is for comfort, not to prove a point about letting the fever run. Would've given it sooner knowing what I know now.
